Audit trail

Definition

An audit trail is the gap-free, chronological record of all relevant events in a system - for AI agents: every tool call with arguments and result, every rule change, every approval and rejection with person, channel and timestamp. The trail becomes tamper-evident only when subsequent modifications are detectable.

Auditors distinguish two questions: "What happened?" is answered by any log. "Did a human answer for it, and is the evidence unaltered?" is answered only by an audit trail that includes decisions and is cryptographically chained. Exportability (PDF/CSV) decides whether the trail actually gets used in the audit.
